DUI, Alcohol & Drug Screening
MVD or court-ordered DUI/alcohol/drug screening will utilize the Michigan Alcohol Screening Test or the Drug Abuse Screen Test. The results will determine clinical recommendations for education or treatment if court-ordered. CK Counseling Group will submit all required documentation to the MVD and or court.

Arizona Revocation Substance Abuse Evaluation*
​​Arizona Revocation Steps:
Determine eligibility for revocation

Email Extended Certified MVR and photo ID to: info@ckcounselinggroup.com.
Note: Pull your Extended Certified MVR through azmvdnow.gov and email it in PDF format. -
Email all completed treatment certificates if the DUI(s) are after 2010.

If proof of treatment is unattainable, 36 hours of DUI treatment is required.
After the determination of eligibility

Complete the Arizona revocation application & questionnaire.

Complete screeners/consents in the portal.

Schedule an appointment with the evaluator for final determination.

Out-of-State Revocation
Out-of-State Revocation
The client must be in Arizona and must ensure there is not a lifetime revocation. The client will need to fill out a state-required application (if any) and will need to have proof of completed treatment.
A drug test may be requested.

Back-to-Work Evaluation
Back-to-Work Evaluation
CK Counseling Group can complete and submit most back-to-work evaluations on the same day depending on the compliance of the client. The client will need to show proof of request by an employer and a photo ID.

A primary goal of therapy is to enhance communication skills between partners, helping them express their thoughts and emotions more effectively and understand each other better.
Peer Support
Peer support creates a safe space where individuals feel accepted without fear of judgment, allowing for open conversations and managing mental health conditions and navigating sobriety.
